LIFE St. Joseph of the Pines administers PACE, the Program of All-Inclusive Care for the Elderly, which offers a total solution for older adults who experience health problems that limit their daily activities. By providing complete medical, health and social services in one location as well as in participants’ homes, LIFE enables older adults to live independently. By establishing a partnership with our senior citizens’ families and loved ones, we prevent our seniors from being placed in a nursing home facility with our services.
At our adult day health center we manage all of our participants health care needs, provide all of their medications both prescription and over the counter medicines, provide in home health services and supplies, and have a clinic at our center under the direction of our medical director. In addition to the medical aspect of our participants needs, we provide transportation and offer physical, recreational and occupational therapies and so much more!

We help veterans 55+ who need to enter long-term care. We partner with care coordinators and care providers to help our clients successfully navigate the complexities faced when placing a loved in supportive housing.

Assists veterans with disabilities, members of the Armed Forces and their families. Focus on necessary home modifications, disability related home repairs, homeless/formerly homeless veterans, deployed service members and military children. |
